Slide 3 - MELANOMA I pazienti con malattia limitata alla sede di insorgenza primitiva (stadio I) necessitano di emocromo completo, test di funzionalità epatica e radiografia del torace. Quelli con metastasi ai linfonodi regionali (stadio II) richiedono TC del fegato, RM del cranio e, se necessario, scintigrafia dello scheletro Laboratorio STADIO I Emocromo completo Rx del torace Test di funzionalità epatica STADIO II e III TC dell’addome RM cerebrale Scintigrafia ossea (se indicata)

Slide 5 - LESIONE PIGMENTATA SOSPETTA Biopsia escissionale Melanoma Per lesioni di diametro inferiore o uguale a 2 cm è indicata la biopsia escissionale per una accurata stadiazione microscopica. Una biopsia parziale (a cuneo) non consente una stadiazione microscopica accurata e dovrebbe essere evitata. La biopsia tramite incisione è indicata per lesioni di diametro superiore a 2 cm.
Slide 6 - Melanoma Stadiazione microscopica Il sistema di Clark classifica istologicamente il melanoma primitivo secondo il livello di invasione del derma. Una lesione di I livello (melanoma in situ) è limitata all’epidermide al di sopra della membrana basale, che risulta indenne; è nota anche come iperplasia melanocitica atipica ed è considerata benigna. Una lesione di II livello penetra attraverso la lamina basale del derma papillare. Una lesione di III livello si estende all’interfaccia del reticolo papillare. Una lesione di IV livello invade il derma reticolare, mentre una lesione di V livello penetra nel tessuto adiposo sottocutaneo. La classificazione di Breslow valuta quantitativamente la profondità di invasione o lo spessore della lesione a partire della membrana basale; è semplice da usare e più accurata. I livelli di Clark e la profondità di invasione sono correlati alle metastasi nei linfonodi regionali e alla prognosi.
Slide 7 - Melanoma Escissione ampia L’ampiezza dell’escissione della lesione primitiva dipende più della profondità di invasione che dall’estensione. Per le lesioni del I e del II livello di Clark sono adeguati margini di resezione di 2 cm, per assicurare una bassa probabilità di recidiva locale. Quando i linfonodi regionali sono clinicamente sospetti se ne pratica la dissezione; essi evidenziano metastasi nel 95 % dei casi. Risulta controversa la dissezione elettiva dei linfonodi in pazienti con esame clinico negativo, ma può rivelarsi utile in alcuni casi. (segue)
